Wraparound planning is a dynamic process which is based on individualized, strength based, needs driven planning and service delivery. A wraparound plan is family centered and surrounded by a team that chooses services and providers to best meet the needs of a family in their home, school, and community. Plans are continually evolving based on a family’s developing strengths and evolving needs.
If you are a professional in the social service or educational field and would like to become a Wraparound Facilitator, please contact your Local Area Network (LAN) in the county in which you work.
Current Wraparound Facilitators (or those interested in becoming a facilitator), please go to Forms and directions.
If you are a parent and would like more information about the wrap-around process or services in your community, please contact the Local Area Network (LAN) in the county that you reside.
