Wraparound planning is a dynamic process which is based on individualized, strength based, needs driven planning and service delivery. A wraparound plan is family centered and surrounded by a team that chooses services and providers to best meet the needs of a family in their home, school, and community. Plans are continually evolving based on a family’s developing strengths and evolving needs.
